Practice Structure

Standing out by working closely with stakeholders to assure continuity of care.

It is clear that reliable healthcare is dependent upon an adaptable infrastructure that provides high level care for seniors across a range of service locations.  The ability to navigate this paradigm is key to our success. The delivery and execution of healthcare is evolving.  Healthcare is a challenging environment.  At PHS, we're optimistic about the opportunities ahead, and we're making investments to ensure that we offer exceptional healthcare, education and research today that benefits Americans tomorrow.

The founders of PHS had a passion for geriatric care.  We have built upon that foundation over our many years of operation to manifest a unifying passion to create something of lasting value for the people we work with and for the patients and families we serve. PHS today combines the best of advanced information technology, the commitment of dedicated professionals, and a culture that emphasizes care and responsibility as we serve our increasingly growing senior population.

We are structured to provide the best and most efficient approach possible to managing and delivering care in this challenging setting. In addition to dozens of  practicing clinicians, PHS' structure also importantly includes Regional Nurse Practitioners, a Compliance Department, a Coding Department, a Clinical Documentation Improvement Department, a Risk Mitigation Department, a Clinical Education Department, a Credentialing and Enrollment Department, and a Revenue Cycle Department.  In spite of bringing to PHS full credentials, each professional who joins PHS completes a fast-paced and comprehensive five day orientation, as well as multiple weeks of additional training specific to long term and post-acute care.

As we grow, we're mindful that ensuring the safe and effective care of seniors is paramount to everything we do. As we expand into new markets, we take painstaking steps to insure that PHS Professionals and our corporate partners understand the collaborative model and the critical keys to improved outcomes.

PHS currently has practices in the following states:

Everyone at Provider Health Services (PHS) shares the common vision that our mission is to improve the well-being and quality of life of seniors and their providers. How we accomplish this mission, our very methodology, is what changes everything. A critical element of our different approach is found in our fundamental operational infrastructure: The PHS Foundation of Care.
